Lesson 6.3: Adding an Online Form

 
Add an Online Form if you want users and participants to send you feedback or questions about the Web Site. The information is then stored on the Web Site and is accessible only by the Web Site Administrator. You have the option of automatically emailing completed forms to you or someone else. You can also email the forms to a remote database if you want to capture the results in a database.
 
Make sure you are logged in as a Web Site Administrator and Click the Web Site Manager Button.

     
Scroll to the Pages Option Section.
It is highlighted in purple.

     
Click on Add a Page.

     
  Click the radio button of the type of page you want to add. Click the Online Form radio button.
Here you can choose the type of page you want to add: Publishing Page, Freeform Page, Online Form or Framed Page. To learn more about these types of pages, click here.
 
   
  Type the page name in the Page Name text box.  
     
Type a description of the page in the Page Description text box.
     
Click the appropriate radio button to make it a public or private page.

Note: A public page can be viewed by guests. A private page can be viewed only by participants.]
     
Click the Continue button at the bottom of the page.
After you click continue you may be asked to enter more information (for an online form and a framed page). Follow instructions on the page until you get to a confirmation screen.

   
     

Fill in the information requested about your form.
On this page you can:

  • Give your form a title.
  • Specify the number of questions for your form.
 
  • Specify any introductory text you want displayed at the top of the form.
  • Specify the text for the form "submit" button
  • Choose whether or not to solicit email addresses on your form.
  • Specify an email address for the form to be mailed..

     
     
Click the Continue button at the bottom of the page.

     
Type in each question for your form in the text fields.

     
Click the Continue button at the bottom of the page.

     
View Confirmation Message.
From this page you may visit your new page, add another page, go to the starting page or your Web Site, or return to the Web Site.

     
To exit the Web Site Manager, Click the "Return to Your Site Name Home Page" link on your Web Site Manager Page.
